Yes, a current referral letter from your GP or other Specialist is required if you wish to receive a Medicare Rebate for your consultation and/or procedure fees. Referrals from GPs are usually valid for 12 months and referrals from Specialists are usually valid for 3 months.
Dr Chang triages all new referrals in order of receipt to determine the order of clinical priority in which patients are seen. We are only able to book an appointment once we have received your referral. Once Dr Chang has triaged your referral, we will contact you by telephone to offer you the first available appointment.
When we confirm your initial consultation we will send you a link via SMS and a pdf via email to complete new patient registration information. If you have not completed that information in advance, please arrive 10 minutes before your appointment time to complete the form in the clinic.
Your referral letter, any current relevant investigation results such as Medical Imaging, Hearing Test or Pathology Results, a list of your current medications, Medicare Card/DVA Card and your Private Health Insurance details.
Our Receptionist will provide you with the consultation fee upon booking your appointment, as well as any other additional fees for procedures that may be performed at your consultation as advised by Dr Chang when he triages your referral (e.g. cleaning of the ears, fibre-optic examination of the nose and/or throat). A Medicare Rebate is applicable with most charges and we can process this for you on the day if you have a valid referral.
Fees are payable ob the day following your consultation. We accept EFTPOS, credit cards (Visa and Mastercard) and cash (correct change only). We do not accept American Express. We are not a bulk billing practice.
